Emergency Contact Forms

There are two different forms, RESIDENTIAL & BUSINESS. The forms differ slightly. The department asks that the forms be completed by residents who reside in the Borough of New Hope or business owners who operate a business within the borough. Both forms are used by the police department to update emergency contact information so if an emergency does arise, the department has current information and can contact a resident, business owner, or property owner. This form can be updated at any time when a change has occurred. Forms may be mailed, faxed, or emailed to the department.
